Peer Support Specialist-Case Manager

Job Summary:

The Coastal Horizons Center, Inc. is seeking a highly motivated and experienced Peer Support Specialist-Case Manager to join our PATH Program team. The successful candidate will provide direct services to vulnerable adults experiencing chronic homelessness, utilizing their own history and lived experience to assist individuals in accessing community resources.

Key Responsibilities:
  • Conduct outreach to adults living in outside locations, such as streets, wooded areas, abandoned buildings, or under bridges.
  • Develop relationships with EMS, local shelters, and community partners to advance the program.
  • Provide peer support and case management services to individuals, including assistance with treatment for mental health, housing, and other area needs.
  • Collaborate with the PATH Team Leader to ensure effective program implementation and individualized support.
Qualifications:
  • Personal history of lived homelessness and experience as a consumer of behavioral health services preferred.
  • One (1) year of recovery with mental health or substance use required.
  • PSS Certification/WRAP Preferred.
