Pediatric Palliative Care RN Coordinator

We are seeking a highly skilled and compassionate RN Coordinator to join our Pediatric Palliative Care team. As a key member of our interdisciplinary team, you will play a vital role in coordinating the care of our patients and their families.
Key Responsibilities- Coordinate the activities of the palliative care team, including scheduling meetings and communicating with team members.
- Assess patients and their families to identify their needs and develop individualized care plans.
- Initiate responses to requests for consultation and coordinate care through community settings and hospice.
- Provide education to staff about palliative care and maintain data on all patients referred.
- Participate in quality improvement activities to enhance the delivery of palliative care services.
- Associate's Degree in Nursing.
- California Registered Nurse License upon hire.
- National Provider BLS - American Heart Association within 30 days of hire.
- 2 years RN experience in the acute care setting or transitional care setting.
- Bachelor's Degree in Nursing.
- HPNA Certification (eligibility for certification required) upon hire.
