Pediatric Palliative Care RN Coordinator Job Description

We are seeking a highly skilled and compassionate RN Coordinator to join our Pediatric Palliative Care team. As a key member of our team, you will play a vital role in coordinating the activities of our palliative care team members and organizing our weekly team conference.

Key Responsibilities:
  • Coordinate the activities of palliative care team members and organize weekly team conferences.
  • Act as the primary link to referring staff and physicians seeking our services.
  • Initiate responses to consultation requests, assess patients, and involve other team members as needed.
  • Coordinate care through community settings and hospice.
  • Educate staff about palliative care and maintain data on all patients referred.
  • Participate in quality improvement activities related to palliative care.
Requirements:
  • Associate's Degree in Nursing.
  • California Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • 2 years RN experience in the acute care setting or transitional care setting.
Preferred Qualifications:
  • Bachelor's Degree in Nursing.
  • HPNA Certification (eligibility for certification required) upon hire.
