Pediatric Palliative Care RN Coordinator

We are seeking a highly skilled and compassionate Pediatric Palliative Care RN Coordinator to join our team at Providence Trinitycare Hospice. As a key member of our palliative care team, you will play a vital role in coordinating the activities of our team members and organizing our weekly team conference.
Key Responsibilities:
- Coordinate the activities of the palliative care team members and organize the weekly team conference.
- Act as the primary link to referring staff and physicians who are seeking the services of the palliative care team.
- Initiate the response to a request for consultation, assess the patient, and involve other team members as appropriate.
- Coordinate care through community settings and hospice.
- Educate staff about palliative care and maintain data on all patients referred.
- Participate in quality improvement activities as it relates to palliative care.
Requirements:
- Associate's Degree in Nursing.
- California Registered Nurse License upon hire.
- National Provider BLS - American Heart Association within 30 days of hire.
- 2 years RN experience in the acute care setting or transitional care setting.
Preferred Qualifications:
- Bachelor's Degree in Nursing.
- HPNA Certification (eligibility for certification required) upon hire.
