Pediatric Palliative Care RN Coordinator Job Summary

We are seeking a skilled RN Care Coordinator to join our Pediatric Palliative Care team. As a key member of our team, you will be responsible for coordinating the activities of all palliative care team members and organizing the weekly team conference.

Key Responsibilities:
  • Coordinate the activities of all palliative care team members
  • Organize the weekly team conference
  • Act as the primary link to referring staff and physicians
  • Initiate the response to a request for consultation
  • Assess patients and involve other team members as appropriate
  • Coordinate care through community settings and hospice
Requirements:
  • Associate's Degree in Nursing
  • California Registered Nurse License upon hire
  • National Provider BLS - American Heart Association within 30 days of hire
  • 2 years RN experience in the acute care setting or transitional care setting
Preferred Qualifications:
  • Bachelor's Degree in Nursing
  • HPNA Certification (eligibility for certification required) upon hire
