Pediatric Palliative Care RN Coordinator Job Summary

We are seeking a skilled RN Coordinator to join our Pediatric Palliative Care team. The successful candidate will be responsible for coordinating the activities of all palliative care team members and organizing the weekly team conference.

Key Responsibilities:
  • Coordinate the activities of the palliative care team, ensuring seamless communication and collaboration among team members.
  • Organize and lead the weekly team conference to discuss patient cases and coordinate care.
  • Act as the primary link to referring staff and physicians, providing timely and effective communication and support.
  • Assess patient needs and involve other team members as appropriate to ensure comprehensive care.
  • Coordinate care through community settings and hospice, ensuring continuity of care for patients and families.
  • Educate staff about palliative care principles and best practices, promoting a culture of patient-centered care.
  • Maintain accurate data on patient referrals and participate in quality improvement activities to enhance palliative care services.
Requirements:
  • Associate's Degree in Nursing.
  • California Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • 2 years RN experience in the acute care setting or transitional care setting.
Preferred Qualifications:
  • Bachelor's Degree in Nursing.
  • HPNA Certification (eligibility for certification required) upon hire.
