Position Overview:
The Family Support Services Coordinator is tasked with engaging prospective clients to assess their eligibility for comprehensive home visitation services. This role involves outreach to families identified as at-risk, utilizing the FROG Scale assessment tool to evaluate their level of risk. Responsibilities include gathering necessary income documentation to establish financial eligibility and maintaining detailed records of client interactions in a centralized database.

Key Responsibilities:

  1. Client Engagement:
    Conduct outreach activities, including phone calls and in-person visits, to initiate the Welcome Visit process with families. Schedule and perform Welcome Visits at various locations, including clients' homes, to evaluate their needs for community resources and determine eligibility for home visitation services. Aim to complete a minimum of 20 Welcome Visits monthly.
  2. Data Management:
    Accurately document all outreach efforts, including sending correspondence to families and completing necessary forms. Ensure that all Welcome Visit information is entered into the statewide database within the specified time frames.
  3. Assessment and Support:
    Provide reliable information regarding family situations to supervisors and assist in crisis scenarios, such as locating housing or food resources. Maintain a high standard of customer service and integrity in all client interactions.
  4. Team Collaboration:
    Attend all required staff meetings and training sessions to enhance skills and support team objectives.
  5. Public Health Participation:
    Engage in public health emergency preparedness activities and contribute to health fairs as directed, ensuring community health needs are met.

Qualifications:
A Bachelor's degree in social work, psychology, sociology, child development, family studies, or a related field is preferred; a high school diploma or equivalent is required. A minimum of one year of experience working with families and children, particularly those aged 0-3, is preferred.

Licenses/Certifications:
A valid driver's license and access to a reliable vehicle with current insurance are required.

Skills and Abilities:
Candidates should demonstrate a commitment to working with diverse populations and possess knowledge of child development. Proficiency in technology is essential, along with the ability to maintain confidentiality and manage sensitive information appropriately.
