Position Overview
This role involves engaging with potential clients to assess their eligibility for comprehensive home visitation services. The position requires outreach to families identified as at risk, particularly those referred to the Healthy Families initiative. Utilizing the designated assessment tool, the FROG Scale, you will evaluate and identify levels of risk. It is essential to collect necessary income documentation to establish financial eligibility and to document all client interactions in a centralized database. Additionally, you will inform clients about various community resources, with a focus on the Healthy Families Program, and refer families to appropriate services tailored to their needs. A key aspect of this role is to encourage and assist at-risk families in developing positive parenting skills while promoting the Healthy Families program within the community.

Key Responsibilities

  • Outreach and Engagement:
    Conduct outreach activities, including phone calls and in-person visits, to engage families in the Welcome Visit process. Schedule and perform Welcome Visits at family homes and community locations, assessing their needs for resources and determining eligibility for home visitation services. Aim to complete a minimum of 20 Welcome Visits monthly.
  • Data Management:
    Document all outreach activities related to assigned referrals, including sending correspondence to families and completing necessary forms. Ensure timely entry of Welcome Visit information into the statewide database in accordance with program policies.
  • Confidentiality and Reporting:
    Maintain the confidentiality of family information and provide accurate reports to supervisors. Assist in crisis situations by connecting families with housing, food, and clothing resources. Uphold integrity in all client interactions and file reports as required.
  • Team Collaboration:
    Participate in all assigned staff meetings, training sessions, and group activities to enhance team support and skill development.
  • Public Health Participation:
    Engage in public health emergency preparedness exercises and contribute to public health initiatives, including participating in health fairs and maintaining confidentiality of sensitive information.

Qualifications

  • A Bachelor’s degree in social work, psychology, sociology, child development, family studies, or a related field is preferred; a high school diploma or equivalent is required.
  • Experience working with families and children, particularly those aged 0-3 years, is preferred.

Licenses and Certifications

  • A valid Indiana Driver's License, access to a reliable vehicle, and current automobile insurance are required.

Knowledge, Skills & Abilities

  • Experience and willingness to work with culturally diverse populations.
  • Knowledge of infant and child development.
  • Proficiency with technology is essential.

Working Environment

  • Work involves a combination of sitting, computer work, attending meetings, and some physical activity. There may be exposure to various living conditions and allergens.
