Clinical Care Coordinator

As a Clinical Care Coordinator, you will play a vital role in coordinating admissions and ensuring a seamless transition for patients into our hospital. Your primary responsibilities will include conducting clinical intake assessments, facilitating communication between medical and psychiatric personnel, and performing benefit eligibility checks.
This role requires strong organizational skills, attention to detail, and the ability to work effectively in a fast-paced environment. The ideal candidate will possess a bachelor's degree in a social services field, experience in mental health, and licensure in their area of clinical specialty. CPR and de-escalation/restraint certification are mandatory requirements.
