About the Job:

We are seeking a highly skilled Clinical Care Coordinator RN to join our ambulatory care team. This individual will be responsible for coordinating patient care and ensuring seamless transitions between services.

Key Responsibilities:
  • Coordinate care using the nursing process and multidisciplinary collaboration.
  • Communicate effectively with patients, families, and healthcare teams.
  • Assess patient safety needs and implement strategies to minimize risk.
Requirements:
  • Current State of Illinois Registered Nurse licensure (or equivalent for Indiana).
  • At least one year of nursing experience required.
  • Bachelor's degree in Nursing or higher (for those hired after January 1, 2014).
  • CPR certification.
  • National Certification preferred.
Compensation:

The estimated annual salary for this position is $70,000-$90,000, depending on experience. Benefits include comprehensive health insurance, retirement plan, and paid time off.
