Position Overview:
The Healthcare Support Specialist plays a vital role in assisting individuals by coordinating and attending to their physical, mental, and behavioral health appointments. Under the guidance of the Nursing Education Director, this position ensures effective communication with healthcare providers, family members, and support staff to meet the healthcare needs of those served.

Work Schedule:
This is a full-time role with a standard 40-hour workweek, Monday through Friday.

Essential Responsibilities:
- Facilitate effective communication among individuals, families, employers, service providers, and other stakeholders.
- Provide transportation and support for medical, dental, psychological, and psychiatric appointments by identifying needs, making referrals, and scheduling visits.
- Ensure the safe operation of vehicles and the well-being of individuals during transportation.
- Accompany individuals to appointments to collaborate with healthcare practitioners and advocate for comprehensive treatment options.
- Maintain detailed documentation of significant medical information to create a comprehensive profile of individual progress and needs, ensuring compliance with relevant standards.
- Assist individuals with daily living activities as directed by their program planning team.
- Participate in the Individual Support Planning Team and/or Behavioral Intervention Team as needed.
- Attend staff meetings and training sessions to stay informed about policies and enhance job performance.
- Be available for after-hours or weekend duties to address individual emergencies.
- Perform additional tasks as assigned.

Qualifications:
- High school diploma, GED, or equivalent knowledge required.
- Must be at least 21 years of age.
- Successful candidates will undergo a pre-employment drug screening and random screenings throughout employment, in line with Department of Transportation guidelines for medical transportation services.
- A minimum of one year of experience in the human services field is required.

Preferred Qualifications:
- Valid CNA certification is preferred.
- At least one year of experience in the healthcare sector or a related field is desirable.

Salary:
Starting at $19.23 per hour.
