Job Summary:

Health Plan San Joaquin is seeking a skilled Healthcare Outreach Coordinator to join our team. As a key member of our outreach and education team, you will be responsible for educating the community about our program and services, establishing and maintaining relationships with community partners, and representing HPSJ at approved community meetings.

Key Responsibilities:

Participate in outreach efforts to educate the community about HPSJ's program and services
Establish and maintain relationships with community partners throughout HPSJ's service area
Represent HPSJ at approved community meetings to network, identify potential partnerships, and visibility opportunities
Assist in event preparation, including setup and takedown
Create required documentation, including call logs and other tracking systems
Receive and initiate calls to potential members to provide information, conduct initial eligibility, follow-up on leads, and for other reasons as required

Requirements:

High school diploma or general education degree
One year in a similar position, or equivalent combination of education and experience
Basic knowledge of the eligibility requirements and benefits of Medi-Cal, ACA, and other state-sponsored programs
Basic knowledge of the healthcare issues facing the underserved
Basic knowledge of community resources that assist the underserved
Strong interpersonal skills, including the ability to interact effectively with individuals both inside and outside of HPSJ
Strong oral and written communication skills, including the ability to communicate professionally to diverse individuals and groups
Ability to work independently and as part of a team
Ability to understand and apply straightforward documents, and oral and written instructions
Ability to compare and discern the difference between multiple sets of data or information
Time management and organizational skills, including the ability to use time effectively and efficiently, and meet deadlines
Demonstrates a commitment to HPSJ's strategy, vision, mission, and values
Basic arithmetic skills
Ability to operate standard office equipment efficiently and effectively
Basic skills in Windows, Word, Excel, PowerPoint, and Outlook
Ability to handle confidential information with appropriate discretion
Ability to speak and be understood in English
Proficiency in one of HPSJ's threshold languages may be required, depending on business requirements
