We are seeking a skilled Occupational Therapist to join our Home Health team at ChristianaCare. As an Occupational Therapist, you will play a vital role in supporting our patients to achieve their goals and improve their quality of life.
Key Responsibilities- Collaborate with our multidisciplinary team to develop and implement individualized care plans for our patients.
- Conduct thorough assessments and evaluations to identify patient needs and develop targeted interventions.
- Provide occupational therapy services to patients in their homes, focusing on activities of daily living, mobility, and overall functional ability.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Participate in ongoing education and training to stay current with best practices and evidence-based interventions.
- Master's degree in Occupational Therapy from an accredited program.
- Current licensure as an Occupational Therapist in the state of Delaware.
- At least one year of experience in home health or a related setting.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently and as part of a team, with a focus on patient-centered care.
