About the Role

We are seeking a skilled Occupational Therapist to join our Home Health team at ChristianaCare. As an Occupational Therapist, you will play a vital role in supporting our patients to achieve their goals and maintain their independence.

Key Responsibilities
  • Collaborate with our multidisciplinary team to develop and implement individualized care plans for our patients.
  • Conduct thorough assessments and evaluations to identify patient needs and develop targeted interventions.
  • Provide occupational therapy services to patients in their homes, focusing on activities of daily living, mobility, and safety.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
Requirements
  • 1 year of experience as an Occupational Therapist, with a valid license to practice.
  • Home care experience preferred, but not required.
  • Proof of a valid driver's license and active auto insurance.
