What You'll Do- Provide high-quality patient care and coordinate services to meet the unique needs of each patient
- Collaborate with our interdisciplinary team to develop and implement personalized care plans
- Conduct thorough assessments and evaluations to identify patient needs and goals
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency
- At least one year of experience as a RN, preferably in home health or a related field
- Valid CPR certification and a reliable vehicle for travel
