RN Medical Records Navigator

Kaiser Permanente is seeking a skilled RN Medical Records Navigator to join our team. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and navigation through the healthcare system.
Key Responsibilities:- Assist patients and families in understanding their diagnosis, treatment options, and available resources within Kaiser Permanente.
- Develop and facilitate individualized plans of care in consultation with treating physicians, patients, and their support networks.
- Provide education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care.
- Coordinate care services, including appointments, procedures, and support services, to ensure seamless patient care.
- Monitor patient compliance with treatment plans and assist with referrals to outside resources and care coordination as needed.
- Develop and maintain high-quality evidence-based care processes through patient education and training to achieve optimal patient outcomes.
- Collaborate with external healthcare members regarding inpatient treatment and discharge planning.
- Identify potential patients for clinical trials and coordinate with research coordinators to consent patients.
- Participate in quality improvement initiatives and contribute to the development and implementation of the Nurse Navigator role at Kaiser Permanente.
- Minimum three (3) years of current RN clinical experience in the area of specialty.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to navigate complex healthcare systems and coordinate care services.
- Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Knowledge of disease specialty, treatments, and delivery of Navigator services.
