Job Summary

We are seeking a skilled and compassionate RN Case Manager to join our team. As a key member of our interdisciplinary team, you will be responsible for creating, planning, and directing the hospice care team, utilizing nursing processes of assessment, interventions, implementation, and evaluation. You will effectively interact with all members of the team, as well as with patients and their loved ones, while maintaining the highest standards of care and professional nursing clinical competency.

Responsibilities
  • Develop and implement individualized care plans that meet the unique needs of each patient
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
  • Provide education and support to patients and their families, promoting independence and self-advocacy
  • Conduct regular assessments and evaluations to ensure the highest quality of care
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures
Requirements
  • Current RN license in the State of Georgia
  • 1-2 years of nursing experience, with a preference for hospice or palliative care experience
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Strong time management and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities
