Community Support Services Specialist

The salary for this role is $38,000 (hourly $18.27) based on experience.

Job Summary:
The Community Support Services Specialist will deliver direct assistance, employment training, and suitable referrals for residents within Carrfour Support Housing Programs.
Key Responsibilities:
- Collaborate with residents to perform initial assessments and create individualized Housing Stability Plans (HSP) that outline both short-term and long-term objectives.
- Utilize evidence-based practices in service delivery, including intensive case management, Motivational Interviewing, Harm Reduction, Trauma-Informed Care, Critical Time Intervention, and Housing First strategies to enhance income, promote self-sufficiency, and ensure housing stability.
- Coordinate with community service providers to deliver additional support in areas such as housing stabilization, financial management, community integration, employment training, benefits acquisition, and referrals for substance use, primary, and mental health care.
- Facilitate and instruct daily living skills workshops for residents, both in groups and individually, as specified in each resident's HSP.
- Conduct regular home visits with residents as determined in collaboration with the Program Supervisor.
- Work alongside the property manager to organize community-building activities, including tenant councils and resident meetings, to enhance life skills.
- Provide crisis intervention as necessary under the guidance of the Clinical Director or Program Supervisor.
- Maintain accurate client records and ensure compliance with policies.
- Ensure timely data entry into the Homeless Management Information System (HMIS).
- Complete all required documentation promptly and efficiently.
- Participate actively in quarterly staff meetings to discuss resident progress towards HSP goals and make updates as needed.
- Engage in weekly/monthly supervision sessions.
- Attend mandatory workshops, training sessions, and meetings.
- Cross-train across various programs and departments to support the success of Carrfour Supportive Services.
- Perform other duties as assigned to contribute to the program's success.
Qualifications:
- High School Diploma or GED required; higher education in social work, mental health, psychology, or a related field preferred.
- Experience working with diverse racial, ethnic, and socioeconomic backgrounds is essential.
- Individuals with less than five years of related experience may need to obtain certification as a Behavioral Health Technician.
- Knowledge of community resources is required.
- A valid driver's license and reliable transportation are necessary.
- Flexibility in scheduling and availability for on-call duties is required.
- Bilingual (English/Spanish or Creole/English) is preferred.
Ideal Candidate:
- Individuals with lived experience in homelessness, mental health, and substance use disorders.
- Those with gaps in their employment history or who have been out of the workforce for six months or more.
