Job Overview

The Supportive Housing Case Manager plays a crucial role in delivering housing and support services to individuals and families facing chronic homelessness within the community. This position is grounded in the Housing First philosophy, which prioritizes providing immediate housing solutions without prerequisites such as participation in treatment programs. The ideal candidate will be a compassionate and dedicated professional, eager to work within a supportive environment aimed at assisting at-risk populations.

Key Responsibilities:

  • Deliver comprehensive client support and engagement strategies to address both immediate and long-term needs in a respectful and structured manner.
  • Facilitate outreach and engagement efforts for families and individuals, ensuring their permanent supportive housing requirements are met in collaboration with the Department of Human Services.
  • Assist clients in navigating housing applications and lease agreements with landlords and contractors as necessary.
  • Conduct initial assessments of housing units and support clients in inspections to ensure compliance with program standards.
  • Collaborate with clients to create service plans, establish goals, and implement effective strategies to achieve these objectives, regularly tracking and reporting on progress.
  • Complete bio-psycho-social assessments for clients who have been housed.
  • Perform ongoing home visits to evaluate client needs and provide appropriate referrals.
  • Engage actively with community service providers and partner organizations to identify resources that can support families.
  • Maintain a minimum of four face-to-face interactions with clients each month.
  • Input client information into Housing and Homeless (HTH) databases, generate reports, and process financial assistance checks.
  • Organize and lead weekly meetings based on client evaluations.
  • Keep thorough, organized, and up-to-date records for each client in accordance with DHS guidelines.
  • Communicate any issues related to client case management to the Team Lead and Program Manager.
  • Provide necessary data for weekly and monthly reporting.
  • Participate in all required training sessions mandated by the company and DHS.
  • Attend scheduled supervision and office days as required.
  • Assist clients with Activities of Daily Living (ADL) skills as needed.
  • Perform other relevant duties as assigned.

Qualifications:

  • Two to six years of experience in the homelessness sector (required).
  • Bachelor's Degree.
  • Experience in crisis management (preferred).
  • Strong written and verbal communication skills.
  • Effective time management abilities.
  • Exceptional multitasking and organizational skills.
  • Proven case management skills with a history of successful outcomes.
  • Understanding of the factors contributing to homelessness.
  • Experience collaborating with landlords and familiarity with local rental markets.
  • Ability to thrive in a team-oriented setting.
  • Capability to work with and serve a diverse population.

Additional Information:

Salary Range: $52,000 - $55,000
