We are seeking a highly skilled Housing Case Manager to join our team at Compass Career Solutions. As a key member of our staff, you will be responsible for providing comprehensive support services to clients in need of housing assistance.
Key Responsibilities:- Client Assessment and Eligibility Determination: Conduct thorough assessments and interviews with clients to determine their eligibility for housing support services.
- Case Management and Planning: Develop and update action plans, case notes, and other documents related to a client's file to ensure effective case management and support.
- Life Skills Development and Support: Provide ongoing life skills development and support to assist clients in improving their independence and quality of life.
- Intervention and Referral Services: Provide specialized and targeted interventions when clients are at risk of eviction, and connect them to medical, mental health, and other supportive counseling services as needed.
- Benefits Navigation and Application Support: Guide clients through the process of application, qualification, and re-determination for SSI, SSDI, Medicaid, Medicare, SNAP, and other services as required.
- Home Visits and Community Outreach: Conduct visits in clients' homes and enlist the support of property management as necessary to ensure client well-being.
- Outcome Tracking and Reporting: Track and report outcomes, providing data updates as directed to ensure effective program evaluation and improvement.
- Professional Development and Representation: Attend and actively participate in scheduled internal and external meetings, and represent Compass Career Solutions at professional meetings, community events, and to members of the community.
Requirements: Ability to use assigned technology as directed in the community, including laptop, cell phone, and designated software.
