Job Title: Residential Registered Nurse RN

We are seeking a skilled and compassionate Registered Nurse to join our team as a Health Care Coordinator at Communitas, Inc.

About the Role

The Health Care Coordinator will be responsible for providing high-quality healthcare services to individuals residing in our community group homes and independent apartment settings.

Key Responsibilities:

  • Provide oversight of the overall healthcare and well-being of individuals in our care
  • Develop and implement individualized care plans to meet the unique needs of each resident
  • Collaborate with healthcare professionals to ensure seamless care coordination
  • Provide education and training to staff on healthcare protocols and procedures
  • Conduct regular assessments to identify and address any healthcare concerns
Requirements

To be successful in this role, you will need:

  • A current Massachusetts license as a Registered Nurse (RN)
  • At least 3-5 years of experience in a community health setting, preferably with adults with developmental and intellectual disabilities
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable transportation
