Key Responsibilities:- Conduct thorough assessments of patients to identify their unique needs and develop personalized care plans.
- Collaborate with physicians to ensure seamless care coordination and effective management of patient plans.
- Evaluate patient progress regularly and make adjustments to care plans as needed to meet evolving needs.
- Provide ongoing education and support to patients and their families to empower them in their care journey.
- Initiate preventive and rehabilitative nursing procedures, referring patients to community services as necessary.
- Document patient progress accurately and timely, ensuring comprehensive and relevant information for care teams.
- Coordinate services, informing physicians, management, and staff of changes in patient condition and needs.
- Determine the scope and frequency of services required based on patient acuity and needs.
- A strong commitment to delivering exceptional patient-centered care.
- Graduate of an accredited nursing school; RN licensure in Rhode Island and Massachusetts preferred.
- Minimum 1-2 years of clinical experience, with a focus on hospice or palliative care.
