Job Title: Patient Care Coordinator

We are seeking a detail-oriented and organized Patient Care Coordinator to coordinate and manage the scheduling of infusion therapy services for our patients.

Key Responsibilities:
  • Office Administration:
    • Answer phones and respond to inquiries in a professional and courteous manner.
    • Manage and maintain accurate and up-to-date patient records in the electronic health record (EHR) system.
    • Help facilitate communication of supply lists between RNs and pharmacies.
  • Scheduling Coordination:
    • Manage and coordinate the scheduling of infusion therapy appointments for patients.
    • Collaborate with nursing staff and other healthcare providers to optimize appointment availability.
    • Help facilitate deliveries by the pharmacies to the patients.
  • Patient Communication:
    • Communicate with patients to confirm appointments, confirm receipt of supplies and drug, and address any non-clinical questions or concerns.
    • Ensure patients are aware of their treatment schedules and any necessary follow-up.
  • Record Keeping:
    • Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.
    • Document scheduling changes and communicate updates to relevant team members.
Qualifications:
  • Education:
    • High school diploma or equivalent required.
    • College degree preferred.
  • Experience:
    • Prior experience in infusion therapy or clinical scheduling preferred.
    • Familiarity with electronic health record (EHR) system, preferably Alayacare.
  • Skills:
    • Strong organizational and time-management skills.
    • Excellent communication and interpersonal skills.
    • Ability to work independently and as part of a team.
Working Conditions:
  • An office-based role.
