**Responsibilities:**

  • Assess patients' financial and social situations to determine eligibility for financial assistance
  • Act as a liaison between patients, families, and public agencies to facilitate the application process
  • Document assessments and approvals in a timely and accurate manner
  • Develop and maintain relationships with community resources and public agencies to ensure access to necessary services
  • Provide exceptional customer service and support to patients and families

**Requirements:**

  • High School diploma or equivalent required; Bachelor's degree preferred

  • Excellent written and verbal communication skills
  • Ability to work effectively in a team environment
