Key Responsibilities- Conduct thorough psychosocial assessments of patients to understand their needs and develop personalized intervention plans.
- Maintain accurate and up-to-date clinical records for all patients referred to social work services.
- Provide information and referral services to patients, families, and caregivers regarding practical and environmental needs.
- Serve as a liaison between patients, families, caregivers, and community agencies to facilitate access to support services.
- Graduation from a bachelor's program in social work accredited by the Council on Social Work Education.
- Licensure as a licensed clinical social worker by the Board of Behavioral Sciences.
- Minimum of one (1) year of experience in healthcare, with preference given to candidates with home health care experience.
- Excellent verbal and written communication skills, along with strong organizational abilities.
- Current CPR certification.
- Valid driver's license and access to a reliable, insured vehicle in good working order.
- Competitive compensation starting at $35-45/hr, commensurate with experience.
- Mileage reimbursement.
- Supportive work environment with opportunities for professional growth and development.
