We are seeking a highly skilled Senior Care Coordinator to join our team at SeaStar Senior Services in Portland, Oregon.

As a Senior Care Coordinator, you will be responsible for providing comprehensive assessments, creating thorough care plans, and implementing care plan actions to reach goals established during assessment and care planning.

You will also be responsible for updating assessments and care plans quarterly or as needed, responding to changes in client's condition or environment, and determining housing needs and assisting in placement for higher level of care or arranging resources for client safety to remain in their home.

Additionally, you will attend care conferences and serve as liaison between facilities, family, and professional fiduciaries, coordinate all medical appointments, specialty follow up visits, procedures, and obtain needed medications, and provide or arrange for transportation to appointments and social outings.

You will also be responsible for documenting all work performed on client's behalf with detailed notes and accurate time or provide monthly reports to responsible parties as desired, and participate in weekly on-call rotation for evenings and weekends.

The ideal candidate will have a Registered Nurse License, experience working with seniors and adults with disabilities, excellent driving record, CPR certified or able to obtain, good observational and writing skills, and excellent listening skills with critical thinking.

We offer a competitive hourly rate of $50.00 per hour DOE, and a part-time, per diem schedule with expected hours of 5 to 10 hours per week to start, which will increase as clients onboard.

We require a nationwide background check and drug test.
