Job Summary

Houston Methodist Willowbrook Hospital is seeking a skilled Clinical Social Work Specialist to join our team. As a key member of our healthcare team, you will provide comprehensive, compassionate, and clinical social work services to patients and their families.

Key Responsibilities
  • Collaborate with physicians and the interprofessional healthcare team to provide sensitive and effective care to patients and their families.
  • Conduct thorough assessments to identify social and emotional aspects of a patient's illness and develop comprehensive treatment plans.
  • Work closely with case managers and other healthcare providers to ensure seamless discharge planning and continuity of care.
  • Communicate effectively with patients, families, and healthcare team members to ensure timely and efficient care.
  • Develop and implement psychosocial treatment plans, including crisis intervention, individual, marital, and family therapies, and patient, family, and caregiver groups.
  • Screen and identify patients and family members who may be victims of abuse, neglect, or domestic violence, and provide necessary interventions and referrals.
  • Stay up-to-date with community resources, healthcare financial and payer issues, and eligibility for state, local, and federal programs.
  • Utilize computer skills, including Microsoft Office Suite, to maintain accurate records and communicate with patients and healthcare team members.
Requirements
  • Master's degree in Social Work (MSW) from an accredited program.
  • Licensed Medical Social Worker (LMSW) or higher license in the State of Texas.
  • One year of hospital social work experience preferred.
  • Ability to communicate effectively with patients, families, and healthcare team members.
  • Knowledge of community resources, healthcare financial and payer issues, and eligibility for state, local, and federal programs.
  • Computer skills, including Microsoft Office Suite.
