Physical Therapist Opportunity at CenterWell Home Health

We are seeking a dedicated and skilled Physical Therapist to join our home health care team. As a member of our team, you will have the opportunity to plan and administer physical therapy treatment for patients in the comfort of their own homes to help them regain their independence and achieve optimal physical function.

  • Conduct comprehensive physical assessments and evaluations to identify patient needs
  • Develop personalized rehabilitation programs to promote patient recovery and independence
  • Document patient progress and treatment plans to ensure effective care
  • Collaborate with healthcare professionals to provide seamless patient care
  • Provide supervision and guidance to support staff to ensure high-quality patient care

Requirements:

  • Graduate of an accredited Physical Therapy Program
  • Minimum of one year of physical therapy experience
  • Current and unrestricted Physical Therapy license
  • Current CPR certification
  • Strong organizational and communication skills
  • Valid driver's license and reliable transportation

Work Schedule:

Full-time, 40 hours per week

Compensation:

$69,800 - $96,200 per year
