The Health & Hospital Corporation of Marion County operates as the public hospital division, offering a comprehensive range of primary and specialty care services at a 327-bed facility along with various outpatient locations throughout the community.


Focus of Adult Outpatient Services:
The Adult Outpatient program is dedicated to addressing the holistic needs of individuals, assisting clients in their reintegration into society.

Accessibility is paramount for those facing mental health challenges, and this initiative aims to provide services in proximity to their homes through community mental health centers.

Available services encompass case management, training for activities of daily living (ADLs), group therapy, limited individual therapy, and sessions with certified peer recovery specialists, alongside comprehensive psychiatric nursing services including medication management and evaluation.

Additionally, the Adult Outpatient program collaborates with the Supported Employment Program (SEP) and Card Shop to equip clients with essential skills for securing and maintaining employment, with ongoing support from SEP consultants.


Role Overview:
The Care Coordinator is responsible for delivering skills training and development to clients based on medical necessity.

Working closely with the treatment team, the Care Coordinator performs essential client assessments and formulates, develops, and updates treatment plans.

This role also involves advocating for clients to ensure they have access to necessary services, which includes fostering community relationships and resources.

Community-Based Role Expectations:
This position is primarily community-based, with most services delivered in clients' homes. The Care Coordinator has the flexibility to create their own community schedule, focusing on specific geographical areas. All clients are located within Marion County, and mileage reimbursement is provided.

The standard schedule for this role is Monday through Friday, from 8:00 AM to 4:30 PM, with one required shift from 10:30 AM to 7:00 PM each week.

Qualifications:
A Bachelor's degree is required. If the candidate holds a Bachelor's degree in social work, they must also possess a current, unencumbered Licensed Bachelor's Social Worker license in the State of Indiana. A valid Indiana driver's license and a driving record that complies with the organization's driving policy are also necessary. What We Offer:

Responsibilities:
  • Embrace and implement the Recovery Model, including AIDET (Acknowledge, Introduce, Duration, Explanation, Thank You) and motivational interviewing techniques.
  • Provide skills training and development to clients in both group and individual settings, as dictated by the person-centered treatment plan.
  • Conduct necessary client assessments and revise treatment plans in collaboration with the treatment team.
  • Deliver crisis management and emergency interventions as needed.
  • Report on client progress and challenges during treatment team meetings.
  • Ensure timely completion of all documentation and compliance with service and billing procedures.
  • Maintain a safe environment through ongoing assessment and intervention as necessary.
  • Provide case management, working with families and community supports to help clients achieve their goals.
  • Participate in continuing education as required.
  • May provide transportation using a company vehicle or personal vehicle as directed.
Knowledge, Skills & Abilities:
  • Basic understanding of behavioral health and substance use disorders.
  • Familiarity with medical terminology and client rights.
  • Proficient in basic computer skills and crisis intervention techniques.
  • Strong interpersonal skills and the ability to communicate effectively.
