Key Responsibilities:
- Deliver weekly case management services in community settings to help clients achieve and sustain housing and benefits.
- Enhance client self-determination through effective illness management strategies.
- Collaborate with community organizations and mental health service providers to coordinate comprehensive care.
- Participate in regular client staffing and program management meetings to ensure alignment with performance metrics and grant obligations.
Qualifications:
- Bachelor's Degree in a health-related discipline with a preference for candidates possessing two years of experience in mental health.
- Possession of a valid driver's license and dependable transportation.
Essential Skills:
- Proven expertise in mental health services and/or co-occurring disorders.
- Strong written and computer skills for accurate documentation.
- Exceptional organizational abilities and proficiency in computer applications.
- Ability to work autonomously, collaboratively, and within community settings.
- Experience in individual, family, and group counseling, along with case management.
- Capability to educate and support clients, families, and community resources.
- Maintain open communication with team members.
- Familiarity with de-escalation techniques.

Core Duties:
- Provide mental health rehabilitation services to individuals and families, aiming to reduce hospitalizations and crisis situations while enhancing community engagement.
- Ensure ethical, legal, and moral continuity of services within a safe and therapeutic framework, adhering to relevant regulatory standards.
- Actively participate as a team member in delivering mental health services, integrating recovery principles into clients' daily lives.
- Offer case management, crisis intervention, skills training, outreach, and group services.
- Complete documentation within established timelines.
- Attend necessary meetings and contribute to quality improvement initiatives.
- Fulfill additional responsibilities as required to support the needs of the homeless and mentally ill population.
Employment Type: Full-time
Compensation: Based on qualifications, experience, and credentials.
