About the Role

This is an exciting opportunity to join our team as a Clinical Care Coordinator - Hospice Services in Miami County and surrounding areas. As a Clinical Care Coordinator, you will be responsible for planning and delivering care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. You will effectively interact with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing.

Key Responsibilities:

  • Plan and deliver care to patients in their homes and facilities
  • Develop and implement individualized care plans that meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure seamless care delivery
  • Document patient care activities and outcomes in accordance with regulatory requirements
Requirements

To be successful in this role, you will need:

  • A valid RN license in the State of Ohio
  • At least 2 years of acute care nursing experience
  • Certification in Hospice & Palliative Medicine (CHPN) preferred
  • Excellent communication and interpersonal skills
  • Able to drive during daytime, nighttime or inclement weather
