Patient Care Coordinator

We are seeking a dedicated Patient Care Coordinator to join our team at SuperCare Health. As a key member of our clinical support team, you will play a vital role in providing exceptional patient care and ensuring the highest level of satisfaction for our patients and customers.
Key Responsibilities:
- Provide personalized patient care and support through effective communication and collaboration with healthcare professionals.
- Gather and maintain accurate patient information, including medical history, treatment plans, and medication lists.
- Assist with ordering and managing medical supplies, equipment, and services.
- Ensure timely and efficient processing of patient orders, including authorizations, insurance verification, and delivery coordination.
- Develop and maintain strong relationships with patients, families, and healthcare providers to ensure seamless care transitions.
- Participate in ongoing education and training to stay current on industry developments, best practices, and company policies.
Requirements:
- Bachelor's degree in a related field or equivalent experience.
- Minimum 1-2 years of experience in a healthcare setting, preferably in patient care coordination or a related field.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Strong organizational and time management skills, with attention to detail and accuracy.
- Ability to maintain confidentiality and handle sensitive patient information.
Compensation and Benefits:
The estimated salary for this position is $25,000 - $35,000 per year, depending on experience and qualifications. Additionally, we offer a comprehensive benefits package, including medical, dental, vision, flexible savings account, 401(k), and voluntary life insurance.
