Care Coordinator Job Summary

We are seeking a skilled Care Coordinator to join our team at Bay Area Community Services. As a Care Coordinator, you will be responsible for developing and delivering high-quality therapeutic interventions and services in support of an individual's wellness and recovery.

In this role, you will work with individuals who are chronically homeless, providing direct, face-to-face services and traveling throughout Solano County. You will be responsible for assessing individual risk status, intervening appropriately, and creating service plans that aim at an individual getting better/healthy and accomplishing service goals.

You will also be responsible for utilizing Evidence-Based Practice for the designated population/program, continuously assessing individual's risk status, and intervening appropriately. Additionally, you will work with outreach teams to determine when to disenroll individuals based on program protocols.

The ideal candidate will have 4 years of paid work experience in mental health/social services, a valid California driver's license, personal vehicle, and proof of personal vehicle insurance coverage. We offer a competitive salary of $27.00 per hour, which increases to $30.00 per hour after satisfactory performance and achievement of all job-related goals and requirements for the 90-day introductory period.
