Key Responsibilities:

As a Clinical Care Transition Specialist, your daily tasks will include:

  • Conducting individual, family, and group counseling sessions.
  • Managing cases and addressing crises effectively.
  • Completing necessary clinical and administrative documentation.
  • Authorizing services as required.
  • Providing outreach services to clients in their homes and other settings to ensure effective treatment and follow-up.
  • Delivering services in accordance with service definitions and state regulations.
  • Offering expert consultation and training to those involved in the care of clients.
  • Conducting clinical screenings, assessments, and interventions based on individual client needs.
  • Providing comprehensive mental health services to individuals, couples, and families.
  • Enhancing communication with community agencies, families, and clients through timely follow-ups.
  • Implementing Behavioral Support Plans and Crisis Plans as outlined in individual care plans.
  • Being available for on-call support during crises, ensuring face-to-face contact within two hours of notification.
  • Documenting all incidents and following up on client concerns.
  • Monitoring compliance with local, state, and federal regulations.
  • Performing additional duties as assigned.

Qualifications:

The ideal candidate will possess:

  • A valid license or provisional license in the state of employment.
  • A Master's degree in social work, counseling, or a closely related field.
  • Preferred experience of at least one year in a behavioral health environment.
  • Strong initiative, judgment, creativity, and analytical skills.
  • Excellent organizational, communication, problem-solving, and customer service abilities.
  • Professional representation of the agency at all times.
  • Flexibility in work schedule and responsibilities to meet changing priorities.
  • Ability to manage confidential information with discretion.
  • Proficiency in Microsoft Word, Excel, and other relevant software.
  • A minimum age requirement of 21 years.
