As a LPN Homecare Specialist, you will utilize the nursing process to assess, plan, implement, and evaluate patient care, ensuring the best possible outcomes for our patients.


Responsibilities:
  • Provide skilled nursing care and services in accordance with prescribed orders
  • Assess signs and symptoms indicating physiologic and psychosocial changes in patients' conditions
  • Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses

Requirements:
  • Currently licensed as an LPN/LVN in the state in which the LPN will practice
  • Current TB or Chest X-Ray
  • Current BLS card
  • One year prior professional nursing experience preferred
