Nurse Care Specialist

The Nurse Care Specialist provides high-quality, patient-centered care to individuals in their homes. This role involves assessing clients' needs, developing personalized care plans, and delivering evidence-based interventions to promote health, wellness, and independence.
Key Responsibilities:
• Conduct comprehensive assessments of clients' physical, emotional, and social needs
• Develop and implement individualized care plans that address clients' unique needs and goals
• Provide skilled nursing care, including wound care, medication management, and vital sign monitoring
• Educate clients and their families on self-care techniques, disease management, and health promotion
• Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
• Maintain accurate and timely documentation of client care and progress
Requirements:
• Current RN licensure in the state of operation
• One or more years of experience in home health care or a related field
• Strong assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
