Emergency Housing Case Manager

The Emergency Housing Case Manager is a critical member of our team at Safe Place for Youth, responsible for providing support and services to youth in need of emergency housing. This role requires a strong understanding of the challenges faced by homeless youth and the ability to connect them with resources and services to help them achieve stability and self-sufficiency.
Key Responsibilities- Process incoming referrals and screen for eligibility
- Coordinate and assist with youth move-ins and exit planning
- Complete intake and program documentation requirements
- Utilize active listening and motivational interviewing skills to support youth in creating a housing and service plan
- Connect youth with resources to address their basic, physical, economic, and psychosocial needs
- Provide housing navigation and support to connect youth with housing resources
- Ensure the health and safety of youth, including crisis intervention and risk assessment
- Document services in relevant databases in a timely and thorough manner
- Bachelor's degree in social science or related field (or equivalent experience)
- 1-2 years of experience providing direct services to high-risk transitional age youth
- 1-2 years of experience providing case management services to high-risk transitional age youth
- Working knowledge of child and adolescent social, emotional, and cognitive development, trauma-informed care, and harm reduction service models
- Flexibility with schedule based on program needs
- Commitment and passion for the mission, vision, values, and culture of Safe Place for Youth
- Prolonged periods of sitting at a desk and working on a computer
- Changes in the environment, such as the office or outdoors
- Physical effort/lifting, such as sedentary- up to 15-20 pounds at times
Mandated Reporting Requirements: The person holding this position is considered a 'mandated reporter' under the California Child Abuse and Neglect Reporting Act and is required to comply with the requirements set forth in CSU Executive Order 1083 Revised July 21, 2017 as a condition of employment.
COVID-19 Vaccination Requirements: Effective August 9, 2021, the State Health Officer has mandated that all workers, paid or unpaid, in homeless services organizations must provide proof of vaccination. If eligible for a booster shot, proof of booster is also required. If not fully vaccinated, weekly COVID-19 testing is necessary depending on the program.
Driver Positions: Applicants are required to provide a copy of their driving record from the Department of Motor Vehicles, along with a copy of their valid driver's license and proof of insurance. Job offers are contingent upon a satisfactory driving record.
