As a Healthcare Navigation Specialist, you will play a pivotal role in enhancing the quality of care for our members. This position involves developing, assessing, and coordinating care management activities tailored to individual member needs, ensuring effective healthcare outcomes.
Key Responsibilities:
- Create and contribute to personalized care plans that address the unique needs of each member.
- Educate members and their families about available services and benefits to improve healthcare access.
- Evaluate member needs and identify barriers to care, recommending resources for optimal outcomes.
- Provide psychosocial support and assist members in accessing local resources such as employment, education, and housing.
- Collaborate with care providers to ensure timely access to services and monitor progress towards care goals.
Qualifications:
- A Bachelor's degree is required, along with 2 to 4 years of relevant experience in a healthcare setting.
- Candidates with clinical licensure must be graduates from an accredited nursing program.
- Strong background in call center operations and medical insurance is preferred.
Skills:
- Excellent customer service skills and the ability to work with diverse populations, including the aging, blind, and disabled.
- Proficiency in Microsoft Office Suite and familiarity with communication platforms such as Avaya and Zoom.
- Bilingual candidates are preferred but not required.
Work Environment:
This role is conducted in a call center setting, where you will assist members through inbound calls, providing education on Medicaid benefits and services.
Performance Expectations:
- Adherence to protocols should be 90% or above.
- Quality of service should meet or exceed 95%.
Training and Development:
New hires will receive comprehensive training, including system access setup and ongoing support from trainers to ensure success in their roles.
