Job Summary

We are seeking a Community Support Coordinator to provide and coordinate community living supports to individuals with disabilities. The successful candidate will be responsible for assisting individuals in making a home for themselves, assisting individuals in making choices, and ensuring that the home is a clean and safe living environment.

Key Responsibilities
  • Provide and coordinate community living supports to individuals with disabilities
  • Assist individuals in making a home for themselves and in making choices
  • Ensure that the home is a clean and safe living environment
  • Assist individuals in maintaining good health
  • Coordinate routine household activities, individual services and supports, and communication and follow through related to specific administrative and personnel matters
  • Work cooperatively with the Program Director and other staff in developing, implementing, and evaluating the outcomes of community living supports provided to individuals with developmental disabilities
Requirements
  • Ability to work cooperatively with individuals with disabilities and their support teams
  • Ability to communicate effectively with individuals with disabilities and their support teams
  • Ability to work independently and as part of a team
  • Ability to maintain confidentiality and handle sensitive information
  • Ability to work flexible hours, including evenings and weekends
