REPORTS TO: Director of Program Services

SUPERVISORY RESPONSIBILITIES: None

ROLE DESCRIPTION: This professional position is dedicated to orchestrating services for adults with intellectual or developmental disabilities. Responsibilities encompass, but are not limited to, the coordination of services addressing medical, psychiatric, social, nutritional, educational, vocational, employment, housing, transportation, recreational, legal, and advocacy needs. Service Coordinators empower individuals to actively participate in and direct their own services. They assist in crafting the most effective support strategies to achieve personal goals. Advocating for individuals receiving support, Service Coordinators also facilitate decision-making regarding the services and supports that individuals desire and require.

QUALIFICATIONS: Candidates should be detail-oriented, capable of managing multiple tasks, and possess strong verbal and written communication skills. Team collaboration is essential. A Bachelor's Degree or equivalent in human services, social work, sociology, or psychology is required, along with a minimum of two years of relevant experience. Work experience may substitute for educational qualifications.

COLLABORATIVE RELATIONSHIPS: Engage closely with Case Managers and agency personnel.

KEY RESPONSIBILITIES:

  • Coordinate and oversee Individual Support Plans (ISP).
  • Conduct assessments with individuals receiving support or coordinate with Direct Support Professionals for assessments as requested by the Conflict Free Case Manager (CFCM).
  • Participate in ISP meetings to contribute to the development of comprehensive, person-centered plans.
  • Draft ISP Programs.
  • Align assessment results with the Outcomes/Action plans of the ISP.
  • Assist with medical appointments as necessary.
  • Identify rights restrictions and facilitate due process for all rights restrictions.
  • Participate in and monitor Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC) reviews as appropriate.
  • Review all incident reports (medical, behavioral, critical) and ensure proper follow-up.
  • Notify family members, guardians, CFCMs, and other advocates regarding changes in condition, behavior, treatment, or service plans.
  • Complete necessary documentation of services, including data input, reports, and plans into Therap.
  • Gather documentation and address SMART Reviews.
  • Ensure completion of all required federal, state, and agency forms (e.g., EA240; Housing; Social Security; Therap updates (IDF), etc.).
  • Prepare/Complete ICAP as needed.
  • Prepare for NCI, CQL, ARSD Surveys as necessary.
  • Engage in Abuse/Neglect Investigations as requested.
  • Perform other duties as assigned by the supervisor.

Frequent Actions: Lifting over 10 lbs., seeing, sitting, standing, reaching, handling, pinching, repetitive movements, talking, and hearing.

Occasional Actions: Kneeling, crouching, walking, lifting over 25 lbs., pushing, pulling, bending, carrying, and balancing.

Seldom Actions: Lifting over 50 lbs.
