Senior Living Care Specialist

Preston Greens Senior Living is seeking a Licensed Practical Nurse (LPN) to join our community.
As an LPN, you will provide nursing care in accordance with the client's plan of care, including comprehensive health and psychosocial evaluation, monitoring of the client's condition, health promotion, and prevention coordination of services. Our ideal candidate has at least two years of LPN experience, preferably in assisted living or nursing home settings.
Key Responsibilities:
- Provide high-quality patient care in accordance with state and federal regulations
- Collaborate with healthcare teams to coordinate patient services and ensure seamless care delivery
- Maintain accurate records and communicate effectively with patients, families, and colleagues
Qualifications:
- Current LPN license in good standing
- Two years of LPN experience, preferably in assisted living or nursing home settings
- Ability to work nights, weekends, and flexible hours as needed
