Job Summary

As an Activity Leader Aide at Living Legends Health, you will play a vital role in developing, implementing, and evaluating activity programs that cater to the diverse needs and interests of our residents. Your primary responsibility will be to assist the Activity Director in creating a comprehensive care plan that promotes the physical, emotional, and social well-being of our residents.

Key Responsibilities
  • Assist in selecting safe and suitable spaces, equipment, and supplies for activity programs that are person-centered and culturally diverse.
  • Transport residents to activities, events, meals, and visitations as per their care plan.
  • Be available for scheduled weekend and evening activities at a minimum of two times per month.
  • Carry out group and individual activity programs independently, including setup and cleanup.
  • Possess knowledge of various leisure activities and be able to teach them to residents.
  • Coordinate activities with the patient/resident schedule of other services.
  • Engage residents in therapeutic task groups specialized for residents with dementia-related diseases.
  • Understand and respect residents' diet restrictions and adaptive equipment needs.
  • Assist with cleaning and preparing the dining room for meals (Care Unit only).
  • Provide redirection to residents who are attempting to self-ambulate and alert appropriate staff as necessary.
  • Assist in reviewing the patient/resident's activities participation and revising the plan as necessary with the Activities Director or Assistant.
  • Assist with transportation of patients/residents via Geri/wheelchair to and from programs and provide guidance/direction of ambulatory patients/residents when necessary.
  • Assist with data entry related to daily individual attendance records for patients/residents.
  • Supervise and orient volunteers, newly hired staff, and pastoral care personnel.
  • Assist in developing the monthly activities schedule based on individual and group needs, interests, and capabilities.
  • Assist with posting the current monthly activities calendar where it is accessible to patients, residents, and staff.
  • Possess conflict management skills to avoid disruptive behavior between patients/residents and intervene as needed.
  • Participate in facility CQI meetings as directed by the Activities Director.
  • Maintain resident rights and adhere to HIPAA privacy, security, and confidentiality standards.
  • Follow all established policies and procedures, including universal Precautions and Infection Control techniques.
  • Contribute to facility Continuous Quality Improvement initiatives.
  • Follow safety procedures by keeping areas free of hazardous conditions.
  • Economical and careful with supplies and equipment.
  • Attend all required in-services.
  • Consistently demonstrate dependability and punctuality.
  • Present an appropriate professional appearance.
  • Prepare any other necessary duties as assigned by Supervisor.
  • Exhibit a spirit of teamwork, positive attitude, and conflict resolution among co-workers.
  • Perform duties with measured accuracy and timeliness.
  • Set high standards and strive to exceed expectations.
  • Identify actual or potential problems and devise an action plan for resolution.

  • Interact positively with residents, family members, and visitors under various conditions and circumstances.
  • Promptly report all needs and concerns to the Supervisor or designated alternate.
