Job Overview

The Transitional Housing Program (THP) Coordinator will oversee a caseload of up to 12 young adults who have aged out of foster care, as well as a ratio of 1 to 10 expectant or parenting youth. This role involves collaboration with the youth, their significant others, placement agencies, and additional support personnel to help these individuals achieve their personal and professional objectives.

KEY RESPONSIBILITIES INCLUDE:

  1. Possessing a comprehensive understanding of transitional housing initiatives and resources available for young adults transitioning from foster care.
  2. Conducting initial assessments with placement agencies and youth, clarifying program expectations, and completing necessary intake documentation.
  3. Upon acceptance into the program, gathering intake data and developing a tailored needs and services plan.
  4. Updating and submitting the needs and services plan to the supervisor within a designated timeframe.
  5. Providing weekly consultations to youth, which may encompass discussions about goal progress, life skills training, parenting education, public benefits guidance, housing advocacy, and computer literacy support.
  6. Assisting youth in managing their financial resources, personal belongings, and valuables.
  7. Maintaining accurate records of youth's financial accounts, savings, and budgeting.
  8. Coordinating referrals for additional services, including medical, therapeutic, psychiatric, parenting classes, and job training as necessary.
  9. Documenting youth's progress in various aspects of life, including home, school, and work, as well as any significant incidents or contacts.
  10. Completing special incident reports and other required documentation for the placement agency.
  11. Maintaining communication with placement workers regarding any significant events or incidents.

12. Consulting with the supervisor on incidents, significant issues, and potential terminations, following up as directed.

13. Engaging in weekly individual supervision and case consultation with the supervisor.

14. Ensuring that client files are comprehensive and current.

15. Submitting all reports and documentation to the supervisor for final review.

16. Facilitating training sessions and support groups as directed by the supervisor.

17. Providing on-call crisis intervention and fulfilling the role of duty-worker as requested.

18. Consistently representing Walden Environment in a positive manner.

19. Performing other duties as assigned or requested.

4. Compliance with all requirements set by Community Care Licensing for employees in licensed community care facilities.
