Key Responsibilities- Provide leadership and oversight to a team of social workers, ensuring they have the necessary resources and support to deliver exceptional patient care.
- Develop and implement strategic plans to improve patient outcomes, reduce costs, and enhance the patient experience.
- Collaborate with multiple departments and community stakeholders to integrate services, coordinate care, and enhance working relationships.
- Monitor and assess operations, ensuring compliance with Joint Commission accreditation standards and regulatory requirements.
- Develop and administer programs to meet social service and long-term community care needs of patients.
- Minimum two (2) years of management experience in a healthcare setting.
- Minimum five (5) years of social worker experience, including direct social work experience in healthcare.
- Masters degree in social work.
- Licensed Social Worker (Hawaii) required.
