Job Overview

We are seeking a compassionate and skilled Social Worker to join our team at Ohana Pacific Health. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality support services to our residents, ensuring their psychosocial, emotional, and environmental needs are met.


Key Responsibilities:

  • Conduct thorough assessments of residents' needs and develop personalized care plans.
  • Provide counseling services and coordinate with other healthcare professionals to ensure seamless transitions.
  • Lead abuse prevention programs and investigate allegations of abuse.
  • Collaborate with residents, families, and healthcare teams to ensure a smooth transition from acute care to long-term care.

Requirements:

  • Master's or Doctoral degree in Social Work from a Council on Social Work Education-accredited program.
  • Hawaii State Social Work license.
  • BLS Certification.
  • Minimum 2 years of experience in long-term care, healthcare, or working with the elderly.
  • Knowledge of medical terminology.
