We are seeking a skilled Speech Language Pathologist to join our team at CenterWell Home Health. As a Home Health Speech Language Pathologist, you will play a vital role in providing high-quality care to patients in their homes.
Key Responsibilities- Evaluate and treat patients with speech, language, and swallowing disorders in a home health setting.
- Develop and implement individualized plans of care to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
- Provide education and training to patients and their families on communication strategies and techniques.
- Document patient progress and communicate with healthcare teams to ensure seamless care coordination.
- Master's degree in Speech Language Pathology or related field.
- Current certification in Speech Language Pathology (CCC-SLP) or Audiology (CCC-A) from ASHA.
- Minimum of six months experience as a speech therapist/speech language pathologist.
- Home health experience preferred.
- Current and unrestricted license to practice in the state of employment.
- Current CPR certification.
- Good organizational and communication skills.
- A valid driver's license, auto insurance, and reliable transportation.
