At Sunbelt Staffing, we are seeking a skilled and compassionate Registered Nurse (RN) to fill a contract position in Home Health care. This opportunity is ideal for a dedicated and experienced nurse who is passionate about delivering high-quality care in the home setting.
Job Summary:We are seeking a Registered Nurse to provide home health care services to patients in the Farmington area. The ideal candidate will have a minimum of 1+ years of nursing experience and be proficient in OASIS. The successful candidate will be responsible for delivering high-quality nursing care, conducting initial patient assessments, and developing customized care plans in coordination with healthcare teams.
Key Responsibilities:- Deliver high-quality nursing care in the home setting, following established care plans.
- Conduct initial patient assessments and develop customized care plans in coordination with healthcare teams.
- Administer medications, provide wound care, and manage IV therapy as needed.
- Educate patients and families on disease processes, treatment options, and home care best practices.
- Maintain accurate and timely documentation of patient care in the electronic medical records (EMR) system.
- Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
- Communicate effectively with physicians, therapists, and other care providers to ensure comprehensive care.
- Adhere to all safety protocols and guidelines in delivering home health services.
- Current and unrestricted RN license.
- Previous geriatric home health experience.
- Strong OASIS proficiency.
- Experience with HCHB a big plus.
- Strong clinical assessment and care planning skills.
- Familiarity with EMR systems and documentation standards.
- Ability to work autonomously and adapt to changing patient needs.
- Excellent organizational and time management abilities.
- Reliable transportation and a valid drivers license.
