Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities:
  • Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
  • Develop and implement individualized care plans to meet the unique needs of each patient, focusing on rehabilitative, preventative, and restorative care.
  • Conduct thorough assessments, identify patient needs, and develop effective interventions to promote optimal health outcomes.
  • Collaborate with the healthcare team, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and adherence to care plans.
  • Document patient progress, including visit notes, assessments, and care plans, in a timely and accurate manner.
Requirements:
  • Graduate of an accredited Licensed Practical Nursing program or vocational nursing school.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
