Physical Therapist

As a Home Health Physical Therapist with CenterWell Primary Care, you will play a vital role in helping patients recover from injuries, illnesses, and surgeries. Our team is dedicated to providing personalized, comprehensive care that enables patients to regain their independence and live healthier lives.
Key Responsibilities- Plan and administer prescribed skilled physical therapy treatment and training for patients.
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function.
- Develop and implement conditioning and rehabilitation programs consistent with physicians' plans of treatment.
- Adjust treatment as needed to achieve maximum results.
- Confer with physicians and clinical team members to obtain additional patient information and develop treatment plans.
- Supervise and evaluate the performance of Physical Therapy Assistants and Home Health Aides.
- Accurately and thoroughly document patients' care observations, interventions, and evaluations.
- Degree from an accredited Physical Therapy Program.
- Minimum of one year physical therapy experience preferred.
- Home Health experience a plus.
- Current and unrestricted Physical Therapy licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
