Care Transition Specialist

At Waterbury Hospital, we are seeking a skilled Care Transition Specialist to join our team. This rewarding role offers the opportunity to make a meaningful impact in the lives of patients as they transition from acute care settings to aftercare facilities.
As a Care Transition Specialist, you will be responsible for coordinating patient transfers, ensuring seamless transitions, and providing exceptional support to patients and their families. Your expertise in hospital operations and knowledge of community resources will be invaluable in helping us deliver high-quality care.
About the Role:
- Salary Range: $60,000 - $80,000 per year, depending on experience
- Location: Waterbury, CT
- Job Type: Full-time
Responsibilities:
- Accept and coordinate incoming referrals for Endocrine and Bone Health services
- Contact patients to schedule consult appointments and ensure timely follow-up
- Utilize Allscripts worklist for referral communications and updates
- Verify insurance eligibility and confirm active referrals
- Run insurance eligibility checks through Eligibility Tracker or insurance websites
- Closure of quality and reporting measures
- Assist with phone calls and office responsibilities as needed
